Students who will be enrolling in preschool for the first time, kindergarten, and sixth grade in August 2021, will be required to have a physical examination in accordance with the requirements of the State of Illinois.
Students who will be entering kindergarten, second and sixth grade need an updated dental exam on file.
Students entering preschool for the first time, in addition to a physical, need their immunizations up-to-date. Lead Screening is required for preschool students; please submit proof showing that the lead test was drawn. The TB portion of the physical also needs to be completed. If your child's doctor does not recommend a TB skin test, please have him/her mark the TB portion of the physical exempt.
Kindergarten students, in addition to the physical and dental exams, must have a comprehensive vision exam on file that took place within one year prior to kindergarten. Lead screening is only required for kindergarten students if no previous screening was done in the preschool program. The TB portion of the physical also needs to be completed. If your child's doctor does not recommend a TB skin test, please have him/her mark the TB portion of the physical exempt. All immunizations need to be up-to-date.
For the 2021-22 school year, all students entering sixth grade, in addition to a physical and dental exam, will need their immunizations up-to-date. Your child will be required to show proof of one dose of Tdap vaccine (combined Tetanus, Diptheria, and acellular Pertussis) regardless of the interval since the last DTaP, DT, or Td dose. They will also need to receive one dose of Meningococcal Conjugate vaccine (MCV4). If your child has already received their updated vaccines, you will need to show documentation.
It is the parents responsibility to make sure these requirements are met prior to the start of the new school year. Your child will be excluded from school on the first day of classes in August if the physical and appropriate immunizations are not completed. Please make your appointment now to avoid the rush.
